Are you ready to start your journey with Brain Care Centre?
In order to access our Concussion Services, you must have a referral from a medical professional.
A properly completed referral needs:
- A completed Referral for Concussion Services Program form filled out by a healthcare professional. We do not accept self-referrals for the concussion program.
- Accompanying medical documentation verifying the diagnosis of concussion (e.g. discharge summary, neuropsychological assessment, MRI/CT report).
** Individuals must be at least 3 months and less than 5 years post-injury to be eligible for concussion service.
If you do not have medical documentation of the injury, please indicate on the form and send in your referral form without it. Brain Care Centre will work with you to obtain the documentation.
